  • are pregnant
  • have certain medical conditions
  • are living in a long-stay residential care home or other long-stay care facility

Our Health Partnership

We are pleased to announce that The Meadows Medical Practice is now part of Our Health Partnership

OHP

registrationNew Patient Registration

The doctors welcome new patients who live within our practice area.  As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and we may ask you to book an appointment with the nurse for a new patient health check. This is an important part of the registration process as it helps us to understand your health and medication needs should you need to see a doctor or order a repeat prescription.

Medical treatment is available from the date of registration but please note that we always prefer the nurse to have completed the new patient health check before any medication is prescribed. Whilst no patient will ever be denied their medication should it be required at short notice you may only be prescribed sufficient medication to 'carry you over' until your check can be completed.

You can check you live inside our practice area using the Postcode checker

Please contact our reception for further information.

PLEASE NOTE THAT YOU CANNOT CURRENTLY REGISTER ON LINE.

If you wish to register as a new patient please complete a new registration form and return to the Practice. A new patient registration forms can be obtained from the surgery.
